STORED PAYMENT CARD AUTHORIZATION TERMS AND CONDITIONS

Effective Date: February 19^th, 2020

PLEASE READ THIS AGREEMENT CAREFULLY. IT SETS FORTH THE LEGALLY BINDING TERMS AND CONDITIONS FOR YOUR PARTICIPATION IN RADIQAL'S SERVICE FOR ONSHORE PURCHASES WITH PARTICIPATING MERCHANTS USING YOUR MEDALLION DEVICE.

IF YOU DO NOT AGREE TO THESE TERMS, DO NOT CHECK THE BOX. IF YOU DO NOT CHECK THE BOX, YOU WILL NOT BE ABLE TO PURCHASE PRODUCTS FROM RADIQAL PARTICIPATING MERCHANTS.

Introduction and Overview

This Stored Payment Card Authorization ("Authorization") allows You to store Your credit or debit card credentials and other information (collectively, "Your Stored Card") with Radiqal Gaming, LLC, operator of MedallionPay™ Shoreside ("Radiqal," "we," "us" and "our") for your convenience in making purchases at third party merchants that participate in an arrangement with Radiqal to accept the Medallion® device ("Medallion device") provided by Carnival Corporation ("Carnival") to initiate transactions that are charged or debited as applicable to Your Stored Card (collectively, "Participating Merchants"). "You" and "Your" in this Authorization refer to the individual who is the cardholder or an authorized user of Your Stored Card, and who provided Your Stored Card to Radiqal for storing under this Authorization.

Please keep in mind that the Participating Merchants are onshore merchants that have contracted with Radiqal. These merchants are separate from any third party merchants located on Carnival vessels or in other Carnival facilities that accept the Medallion mobile application (“Medallion App”) and have contracted with Carnival. Your Stored Card under this Authorization is not stored with Carnival (and instead is stored with Radiqal), and Carnival has not entered into contracts with Participating Merchants.

By checking the box, You agree to all of the terms and conditions of this Authorization. An e‑mail confirmation will be sent to You at the most recent e-mail address You have provided to Radiqal. Please keep in mind that, by making a purchase using the Medallion device at Participating Merchants, You also agree to Radiqal’s terms of use at https://medallionpay.com/terms/customer/.

You authorize Radiqal to store Your Stored Card. You represent and warrant to Radiqal that You are the cardholder or an authorized user of Your Stored Card. Your Stored Card is kept confidential and secure by Radiqal in accordance with Radiqal’ s Privacy Policy at https://medallionpay.com/privacy/customer. If You wish to make any changes to Your Stored Card, please log into your Carnival App account and select OceanProfile™ -> My Account -> Payment Methods.

We will use Your Stored Card when you make purchases by using the Medallion device at Participating Merchants to initiate charges or debits to Your Stored Card rather than you having to present the physical plastic card. The amount of each transaction using Your Stored Card will be the purchase price of the good or service purchased from the Participating Merchant, including any associated taxes, shipping charges, duties or other charges. You will be provided by each Participating Merchant with a receipt showing the transaction amount, including the transaction currency, for each transaction initiated through the Medallion device at a Participating Merchant.

All purchases made by use of the Medallion device at Participating Merchants to initiate charges or debits to Your Stored Card are subject to the Participating Merchant’s policy. Please inquire with the Participating Merchant regarding their refund or exchange policies prior to making a purchase.

You acknowledge and agree that, although the Radiqal program involves use of the Medallion App, Radiqal is solely responsible for storage of Your Stored Card, and Carnival has no responsibility or liability with respect to such storage or any other matters arising out of or relating to this Authorization, including without limitation any purchase or other transactions You may enter into with Participating Merchants. You agree that Carnival is a third party beneficiary of this provision of this Authorization.

We will notify You of any changes to this Authorization by e-mail messages sent to the most recent e-mail address You have provided to us. You may cancel this Authorization by deleting Your Stored Card from the Medallion App under the payment card section. This Authorization will remain in effect until cancelled by You or us. Your consent to this Authorization will be retained by Radiqal.

By checking the box when storing a card, You authorize Radiqal to store Your Stored Card and agree to the terms and conditions of this Authorization, including the disclaimer of responsibility and liability by Carnival.
